Making a Subject Access Request
Caterham High School needs to collect and use certain types of information about students, parents, staff and other users. The Data Protection Act and GDPR gives anyone the right to find out about the information held about them.
How to Access Your Information
To access your information you will need to make a Subject Access Request. You can do this by:
- Downloading the Subject Access Request Form
- emailing your request to email@example.com
- sent a letter to the Data Protection Lead at the schools address
Once we have received your request we will require you to complete the Subject Access Request Form if not already completed and return this to us along with proof of identity, we must make sure you are who you say you are. We will not require proof of ID for current members of staff and students. Once we recieve the form along with proof of ID we will start the search for information.
If you are making a request regarding a child in your care we will need proof that you hold parental responsibility in law by providing a copy of the full birth certificate for the child on which you have been names.
If the child is aged 13 or over we will require them to sign a consent to share form to allow access to their personal data.
If you require any further information or have any queries relating to how the school processes data please contact the schools Data Protection Lead, Melinda Gooding on 020 8551 4321 or email firstname.lastname@example.org.
